Job Title: Physical Health Nurse

We are seeking a skilled and compassionate Physical Health Nurse to join our team at Elysium Healthcare. As a Physical Health Nurse, you will play a vital role in providing high-quality care to our service users, working collaboratively with the multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities:
  • Lead and coordinate all aspects of physical health needs for service users, including monitoring observations and assessments to identify areas for improvement.
  • Develop and support physical health interventions, adhering to the NMC Code of Conduct and local, Trust, and national standards of practice.
  • Engage actively in practice development, evaluation, and audit activities to promote evidence-based practice.
  • Work collaboratively with primary health services, developing and maintaining links with GP practices and other physical health departments.
  • Monitor and maintain physical health dashboards to track progress and identify areas for improvement.
Requirements:
  • A relevant nursing qualification and registration.
  • Phlebotomy skills.
  • Strong team-working skills and a high level of self-motivation.
  • A positive attitude and a natural ability to care for and support others.
  • A commitment to high-quality care and evidence-based practice.
