About the Role

We are seeking a highly skilled and experienced Physical Health Nurse to join our team at Elysium Healthcare. As a Physical Health Nurse Specialist, you will play a key role in leading and coordinating the physical health needs of our service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team.

Key Responsibilities

  • Monitor observations and assessments to identify any actions that need to be addressed and documented.
  • Develop and support physical health interventions used in the clinical area, adhering to the NMC Code of Conduct and facilitating compliance with local, Trust and national standards of practice.
  • Engage actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
  • Work collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
  • Monitor and maintain Physical Health Dashboards.

Requirements

  • A relevant nursing qualification and registration.
  • Phlebotomy skills.
  • Strong team-working skills.

  • Understand the physical health care needs for individuals who are living with enduring mental health difficulties.

About the Team

The Woodmill is an 18 bed rehabilitation service for women with complex emotional and mental health needs. Our team is dedicated to providing high-quality care and support to our service users, and we are looking for a skilled and experienced Physical Health Nurse to join us.
