Specialist Wellbeing Nurse

We are seeking a highly skilled and motivated Specialist Wellbeing Nurse to join our Integrated Health and Social Care team. As a key member of our team, you will play a pivotal role in providing multidisciplinary working and enhancing care delivery to our patients.
Key Responsibilities- Provide holistic patient-centered care, utilizing your expertise in wound management, pressure ulcer prevention, palliative, and end-of-life care.
- Develop and implement care plans, working collaboratively with the multidisciplinary team to ensure seamless care delivery.
- Lead by example, demonstrating exceptional leadership qualities, and mentoring junior staff to enhance their skills and knowledge.
- Contribute to the development of the team, sharing your expertise and knowledge to drive excellence in care delivery.
- Participate in regular supervision and support, with ongoing professional development opportunities to enhance your skills and career progression.
