Key Responsibilities- Deliver high-quality patient care in the community, including assessments, diagnoses, and treatment plans.
- Work collaboratively with patients, families, and healthcare professionals to promote health, prevent illness, and manage long-term conditions.
- Conduct home visits and provide care in patients' homes, as well as in community clinics and other settings.
- Develop and implement individualized care plans, taking into account patients' needs, preferences, and goals.
- Collaborate with other healthcare professionals, including GPs, district nurses, and social workers, to ensure seamless care coordination.
- Participate in multidisciplinary team meetings and contribute to the development of care plans and strategies.
- Stay up-to-date with the latest evidence-based practice and guidelines, and apply this knowledge to improve patient outcomes.
- Contribute to the development and implementation of policies and procedures, and participate in quality improvement initiatives.
- Registered Nurse with a valid NMC registration.
- Experience in community nursing, preferably in a similar role.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
- Ability to maintain accurate and detailed records, and to contribute to the development of care plans and strategies.
