Job Title: Care Coordination Specialist

We are seeking a dedicated and experienced Care Coordinator to join our team. The primary focus of this role is to ensure that clients receive the highest quality of care and support workers are well-supported and motivated.

About the Role

The Care Coordinator will act as the primary point of contact for clients and support workers, providing critical operational support and overseeing daily care logistics. Key responsibilities include developing and managing care schedules, monitoring and addressing cancelled care hours, overseeing high-quality, person-centred care, hosting team meetings, recruiting and matching support workers to clients, collaborating with HR and Clinical Leads, coordinating with internal and external stakeholders, supervising care and medication standards, and planning and deploying resources efficiently across care packages.

Main Duties
  • Develop and manage care schedules to meet clients' needs, ensuring coverage for staff absences.
  • Monitor and address cancelled care hours with Team Leaders.
  • Oversee high-quality, person-centred care and maintain strong communication with clients and families.
  • Collaborate with HR and Clinical Leads to address staff issues and ensure ongoing training.
Requirements
  • Strong customer service skills with experience in domiciliary care or a similar environment.
  • Outstanding communication and logistical planning skills.
  • Mentoring and coaching experience to support the development of other team members.
