Position Overview

This role is situated within the Community Care sector, focusing on the assessment, management, and treatment of a diverse group of clients with intricate and/or multiple health requirements. The successful candidate will collaborate closely with the multidisciplinary team (MDT) and provide guidance to therapy assistants, junior personnel, and students. There may be a requirement to operate across various locations within the organization.

Note: Previous applicants within the last three months are not eligible to reapply.

The physiotherapist will be part of the 7-day urgent community response rota and may be required to work independently in accordance with the lone working policy.

This is a permanent position.

Key Responsibilities

We seek a dedicated, enthusiastic, and adaptable physiotherapist to join our supportive and dynamic team in the Community Care sector.

Our services include both rapid and planned responses, facilitating early discharges from healthcare facilities while aiming to minimize hospital admissions. We also provide support for discharge-to-assess beds.

The role encompasses a varied caseload, requiring demonstrable knowledge, experience, and relevant clinical skills in managing patients with complex or long-term conditions, including falls, orthopaedic issues, general mobility challenges, and neurological disorders.

The successful candidate will function as an integral member of our multidisciplinary team, collaborating with District Nurses, Advanced Nurse Practitioners, Complex Case Practitioners, and MacMillan Nurses, while also supervising junior staff and students.

Administrative responsibilities are part of the role, necessitating proficiency in computer use, particularly with Microsoft applications, along with accurate typing skills. Our team leverages digital technology to support remote working and promote a flexible working environment, with access to a laptop and mobile phone.

A valid driving license and access to a personal vehicle are required.

Clinical Responsibilities

As a specialist health and care professional, you will be expected to:

  • Manage a clinical caseload independently, without direct supervision.
  • Conduct comprehensive physiotherapy assessments and develop therapeutic diagnoses, implementing agreed treatment plans for complex clients.
  • Provide individual or group therapy as appropriate.
  • Monitor and evaluate treatment effectiveness, ensuring service delivery meets established standards.
  • Develop care programs to facilitate client discharge.
  • Ensure the safe use of clinical equipment and provide training to others.
  • Maintain accurate and comprehensive documentation in accordance with legal and local standards.
  • Collaborate with the MDT and participate in case reviews and meetings.
  • Contribute to safeguarding clients and assess capacity for informed consent.
  • Maintain professional registration and adhere to organizational guidelines.

Leadership and Management

In this role, you will also be responsible for:

  • Effective communication with various professionals within the organization and external agencies.
  • Assisting in the planning and development of therapy services.
  • Participating in departmental objectives and supporting a collaborative leadership style.
  • Providing training and support to junior staff and students.
  • Managing workload allocation within your specialty area.

Education and Development

As a specialist, you will be expected to:

  • Organize and lead training programs for junior therapists and assistants.
  • Critically evaluate research and apply findings to practice.
  • Maintain competency through ongoing professional development.

Research and Governance

You will also engage in:

  • Measuring and evaluating your work through evidence-based projects and audits.
  • Participating in local and national meetings to advocate for service improvements.
  • Supporting research initiatives within the service.

Qualifications

Essential:

  • Degree in Physiotherapy.
  • HCPC registration.

Desirable:

  • CSP registration.
  • Experience in a community setting.
  • Completion of a band 5 rotation.

Communication Skills

Essential:

  • Strong written and verbal communication abilities.

Desirable:

  • Proficient IT skills.
