Specialist Community Physiotherapist

Position Overview
We are seeking a dedicated and skilled Specialist Community Physiotherapist to join our dynamic team within the Crewe Care Community. This role involves evaluating, managing, and delivering treatment to a diverse caseload of individuals with intricate and/or multiple health requirements. Collaboration with the multidisciplinary team (MDT) is essential, and the role includes mentoring therapy assistants, junior staff, and students.
Key Responsibilities
- Conduct comprehensive physiotherapy assessments and develop tailored treatment plans for service users with complex conditions.
- Deliver individual or group therapy sessions as appropriate, ensuring the highest standards of care.
- Monitor and evaluate treatment effectiveness, making necessary adjustments to care plans.
- Maintain accurate and up-to-date documentation in compliance with legal and organizational standards.
- Engage in collaborative case reviews and contribute to safeguarding service users.
- Support the MDT by sharing insights and participating in various meetings.
Qualifications and Experience
- A degree in physiotherapy and current HCPC registration are essential.
- Experience in the NHS and community settings is required.
- Desirable qualifications include CSP registration and experience in student education.
