Job Summary

This is a challenging and rewarding opportunity for a skilled Physiotherapist to join our dynamic team in the Community. As a Specialist Physiotherapist, you will be responsible for assessing, managing, and providing treatment to a caseload of service users with complex and/or multiple needs.

Main Duties

We are seeking an enthusiastic and motivated Physiotherapist to join our team in Crewe Care Community. The successful candidate will work closely with the Multidisciplinary Team (MDT) and provide supervision to therapy assistants, junior staff, and students. You will be expected to work between multiple sites in the organisation and join the 7-day, urgent community response rota.

As a Specialist Physiotherapist, you will be responsible for:

  • Managing a clinical caseload with autonomy and working without direct supervision
  • Undertaking comprehensive physiotherapy assessments and providing therapeutic diagnoses
  • Implementing agreed treatment plans and providing individual or group therapy as appropriate
  • Monitoring and evaluating treatment to measure progress and outcomes
  • Developing programmes of care to support discharge from service
  • Recording, maintaining, and storing accurate and comprehensive documentation
  • Communicating with the MDT and other professionals to ensure effective care delivery
Requirements

To be successful in this role, you will need:

  • A degree in physiotherapy and registration with the Health and Care Professions Council (HCPC)
  • Previous experience of working in the NHS and in a community setting
  • Good written and verbal communication skills and good IT skills
