Main Purpose of the Role

As a member of the Local Solutions team, you will work closely with unpaid carers to provide them with the support and resources they need to thrive in their caring roles.

Key Responsibilities
  • To support carers through the Carers Assessment process, using a variety of methods to ensure they receive the best possible care.
  • To develop and implement Carers Support Plans, working closely with carers to understand their unique needs and goals.
  • To provide carers with information and guidance on the range of services available to support them, and to help them access these services.
  • To work collaboratively with colleagues in statutory services to ensure a seamless and effective support system for carers.
  • To develop and maintain relationships with key stakeholders, including carers, professionals, and community groups, to promote the well-being of carers and support their caring roles.
Accountabilities
  • To adhere to data protection legislation and data sharing agreements, ensuring that all carers supported by the service are accurately recorded and followed up on as required.
  • To meet internal and external demands for information, demonstrating compliance with targets, standards, statutory, regulatory, or other requirements.
  • To maintain a high level of customer-focused support, ensuring that all interactions with internal and external parties are professional and effective.
Additional Duties
  • To carry out any other duties as necessary or as requested by the team or management.
  • To participate in professional development opportunities, using relevant learning to improve knowledge and skills.
  • To ensure that all activities of the project provide equality of opportunity for carers.
  • To ensure that all policies and procedures are implemented within working practice.
Requirements
  • Experience of working in health or social care, with a focus on supporting unpaid carers.
  • Experience of working as part of a team, with the ability to prioritize own workload and manage own diary.
  • Technical expertise in using Microsoft Office and Outlook.
  • A car owner/driver is essential for this city-wide post.
