Main Purpose of the Role

As a member of the Local Solutions team, you will work with unpaid carers to provide them with the support they need to access practical and emotional support.

Key Responsibilities
  • To support carers through the Carers Assessment process, using a variety of methods such as home visits and community outreach.
  • To complete Carers Support Plans with carers, using a person-centred approach.
  • To provide carers with information about the range of services available to them and how to access them.
  • To work collaboratively with colleagues in statutory services to ensure that carers receive the support they need.
  • To develop and sustain links with partnership organisations to encourage collaborative working to support unpaid carers.
Accountabilities
  • To adhere to data protection legislation and data sharing agreements, ensuring that all carers supported by the service are accurately recorded.
  • To meet internal and external demands for information to demonstrate compliance with targets, standards, statutory, regulatory, or other requirements.
  • To maintain a high level of customer-focused support when dealing with internal and external parties.
Additional Duties
  • To carry out any other duties appropriate to this post, as necessary or as requested.
  • To participate in professional development, using appropriate learning opportunities, to improve relevant knowledge and skills.
  • To ensure that all activities of the project provide equality of opportunity.
Requirements
  • Experience of working in health or social care.
  • Experience of supporting unpaid carers or working with people.
  • Ability to work as part of a team, prioritise own workload, and manage own diary.
  • Technical expertise in Microsoft Office and Outlook.
  • Enhanced Disclosure & Barring Check and mandatory safeguarding training.
  • Travel is required, so a car owner/driver is essential.
