Job Title: Carers Development Worker

Job Summary:

We are seeking a highly motivated and experienced Carers Development Worker to join our team at Local Solutions. As a key member of our team, you will work closely with unpaid carers to provide support, guidance, and practical assistance to help them access the resources they need to maintain their own health and wellbeing.

Key Responsibilities:

  • Support carers through the Carers Assessment process, providing guidance and assistance to help them access the services they need.
  • Develop and implement Carers Support Plans, working closely with carers to identify their needs and goals.
  • Provide carers with information about the range of services available to support them, and help them to access these services.
  • Liaise with colleagues in statutory services to ensure that carers receive the support they need.
  • Work collaboratively with key stakeholders to raise awareness of carers and promote the referral pathway for carers.
  • Support carers to access the relevant support dependant on their need.
  • Take part in presentations, attend meetings, and provide information at events and activities that will offer support to carers.
  • Provide monthly KPIs and reports as required.

Requirements:

  • Experience of working in health or social care.
  • Experience of supporting unpaid carers or working with people.
  • Ability to work as part of a team, prioritise own workload, and manage own diary.
  • Technical expertise in Microsoft Office and Outlook.
  • Enhanced Disclosure & Barring Check and mandatory safeguarding training.
  • Travel is required, so a car owner/driver is essential.
