Job Summary

NeighborWorks Housing Solutions (NHS) is seeking a skilled Case Manager to join our team in Southern Massachusetts. As a Case Manager, you will play a critical role in promoting the health and well-being of our clients by providing housing resources and education.

Key Responsibilities
  • Implement and facilitate the EOHLC Re-housing Plan program objectives, including budgeting, parenting, time management, nutritional planning, and health and hygiene skills.
  • Implement and facilitate the EOHLC Stabilization Plan for guests leaving shelter.
  • Supervise residential staff.
  • Perform other duties assigned by the Director of Housing Services.
Requirements and Qualifications
  • Relevant case management experience.
  • Knowledge and/or skills to access and/or develop community resources.
  • Demonstrated ability in clear verbal and written communication.
  • Knowledge and/or experience relevant to working with client population.
  • Ability to work independently and cooperatively as a member of a professional team.
  • Proficient computer skills, including the use of Microsoft Word and Excel.
  • Valid Driver's License.
Working Conditions

This position requires the ability to work in a fast-paced environment and to commute between units and providers. A valid driver's license and a reliable motor vehicle are essential for this role.
