**Job Summary**

Wakefield Council is seeking a skilled Care and Support Worker to join our Reablement Service team. As a Care and Support Worker, you will play a vital role in providing high-quality care and support to vulnerable individuals in our district.

**Key Responsibilities**

  • Deliver person-centered care and support to individuals in their own homes, promoting independence and well-being.
  • Work collaboratively with colleagues and other healthcare professionals to ensure seamless care delivery.
  • Participate in comprehensive induction and training programs to develop skills and knowledge.
  • Contribute to the development of individual care plans, ensuring that each person's needs are met.
  • Provide emotional support and companionship to individuals, promoting a sense of belonging and connection.

**What We Offer**

  • A competitive salary of £23,000 - £23,000 (FTE), with enhancements for weekend and bank holiday working.
