To deliver and develop bedside models for patients impacted by serious or multiple injuries, predominantly within major trauma centres.
Main Responsibilities- Work closely with NHS clinical staff and community organisations to address patient needs following catastrophic injury.
- Operate a case management approach, assessing and identifying needs, providing support, signposting, and making referrals.
- Maintain detailed case records, using Day One's CRM system, capturing data in line with policies and regulatory requirements.
- Implement processes for collecting patient feedback and contributing to service monitoring and evaluation.

RequirementsThe successful candidate will have strong communication skills, be able to work effectively in a team environment, and have experience in case management or a related field.

BenefitsThis role offers a competitive salary, between £35,000 and £45,000 per annum, depending on experience.
