Job Summary

To deliver and develop bedside models for patients impacted by serious or multiple injuries, predominantly within major trauma centres.

Main Responsibilities
  • Work closely with NHS clinical staff and community organisations to address patient needs following catastrophic injury.
  • Operate a case management approach, assessing and identifying needs, providing support, signposting, and making referrals.
  • Maintain detailed case records, using Day One's CRM system, capturing data in line with policies and regulatory requirements.
  • Implement processes for collecting patient feedback and contributing to service monitoring and evaluation.
