Job Description

This is an exciting opportunity to join our team as an Adult Care Services Manager at The Confederation, Hillingdon CIC. As a key member of our care coordination team, you will be responsible for providing expert care coordination and support to individuals with health conditions and their families.

Main Duties and Responsibilities:
  1. To develop and implement comprehensive care plans that meet the unique needs of each individual.
  2. To provide expert guidance and support to individuals, their families, and carers in navigating the health and care system.
  3. To work closely with GPs and other primary care professionals to ensure seamless care coordination and delivery.
  4. To assist individuals in accessing relevant self-management education courses, peer support, and other interventions that promote their health and wellbeing.
  5. To communicate effectively with GPs and other professionals involved in an individual's care, ensuring that all parties are informed and engaged throughout the care planning process.
  6. To make and manage appointments, referrals, and other administrative tasks associated with the role.
  7. To maintain accurate and up-to-date records of individual care plans, progress, and outcomes.

In return for your expertise and dedication, we offer a competitive salary of £45,000-£55,000 per annum, depending on experience, plus a range of benefits and opportunities for professional development.
