Key Responsibilities- Provide expert clinical leadership and guidance to nursing staff in the care of older adults with dementia.
- Develop and implement effective care pathways and protocols to improve patient outcomes.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and coordination.
- Conduct comprehensive assessments and develop individualized care plans for patients with complex needs.
- Provide education and training to staff on dementia care and management.
- Registered Nurse with a valid NMC registration.
- Postgraduate qualification in a relevant field (e.g., dementia care, gerontology).
- Non-medical prescribing qualification (V300).
- Proven experience in leading and managing nursing teams.
- Excellent communication and interpersonal skills.
