The Role

We are seeking a Support Practitioner to join our team in Perth and Blairgowrie. As a Support Practitioner, you will play a vital role in supporting individuals to manage their own homes, increase and maintain their independence, and encourage and support them with their social, health, and wellbeing needs.

Your Responsibilities

  • Support individuals to develop personal plans that reflect their choices, individual support needs, and capabilities.
  • Assist individuals to access leisure facilities, engage in meaningful activities, and participate in hobbies and interests that are important to them.
  • Work collaboratively with the team to ensure that individuals receive the best possible care and support.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Contribute to the development and implementation of individual support plans.
  • Monitor and record individual progress, identifying areas for improvement and implementing changes as necessary.

We are looking for a motivated and compassionate individual who is passionate about supporting others. You will have excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and other stakeholders.
