The Role

We are seeking a highly motivated and compassionate Support Practitioner to join our team in Perth and Blairgowrie. As a Support Practitioner, you will play a vital role in providing person-centered support to individuals, enabling them to manage their own homes, increase and maintain their independence, and engage in meaningful activities.

Key Responsibilities

  • Provide support to individuals with disabilities and additional support needs to achieve their goals and live fulfilling lives.
  • Assist individuals with daily living tasks, such as personal care, meal preparation, and household management.
  • Encourage and support individuals to participate in leisure activities, hobbies, and interests.
  • Work collaboratively with the support team to ensure a high-quality service is delivered.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.

Requirements

  • Great communication and interpersonal skills.
  • A positive attitude with a solution-focused approach.
  • Good team working skills and ability to work on your own initiative.
  • Ability to cook and prepare meals.
  • Willingness to learn and use new systems and technology.
  • Ability to apply good administrative and numeracy skills.
