Resident Care Coordinator

We are seeking a dedicated and compassionate Senior Care Assistant to join our team at HC-One. As a Senior Care Assistant, you will be responsible for providing high-quality care to our residents, working collaboratively with the Registered Nurse and Care Manager to develop and implement personalized care plans.
We offer a competitive salary of £21.48 per hour, paid Enhanced DBS/PVG, free uniform, and access to a range of benefits and services to support your physical and psychological well-being. As a Senior Care Assistant, you will assist with medication administration and record-keeping, and develop strong relationships with the rest of your team.
Key Responsibilities:
- Provide person-centered care to our residents, ensuring their dignity and independence are maintained
- Assist with medication administration and record-keeping
- Develop and implement personalized care plans in collaboration with the care team
Requirements:
- Level 2 Diploma S/NVQ in Adult Care or equivalent qualification
- Ability to work towards Level 3 qualification in Adult Care
- Background in person-centered care planning and knowledge of caring for people with dementia
