Dementia Outreach Support Specialist

About the Role
We are seeking a dedicated and compassionate Dementia Outreach Support Specialist to join our team in Nottinghamshire. This is an exciting opportunity to work with clients with dementia in residential/nursing care, providing specialist nursing/healthcare advice, treatment, and education under the guidance of professionally registered team members.
As a member of our multi-disciplinary Outreach service, you will contribute to assessments and interventions carried out by the dementia outreach team, engaging with the Enhanced Health in Care Homes Service (EHCH) and GPs in the local area.
Your main duties will include supporting the implementation of patient care plans, monitoring and recording individuals' mental health status, and reporting any significant changes. You will also assist individuals in developing and maintaining coping strategies to manage their symptoms.
The ideal candidate will have excellent communication skills and be able to work effectively with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia.
Salary and Benefits
The estimated salary for this role is £27,000 - £32,000 per annum, depending on experience. The successful applicant will also receive a range of benefits, including opportunities for career development and progression, a supportive working environment, and access to training and supervision.
