Job Description

As a Community Support Specialist at Achieve together, you will work collaboratively with our team to deliver high-quality care and support services to individuals in their homes and local communities. You will be responsible for:

  • Developing and implementing personalized care plans to meet the unique needs of each individual
  • Providing emotional support, promoting independence, and empowering individuals to make informed decisions about their lives
  • Building strong relationships with service users, their families, and other stakeholders to ensure seamless communication and coordination of care
  • Participating in ongoing training and development to enhance your skills and knowledge in care and support services
Salary and Benefits

We offer a competitive salary of £11.44 - £11.54 per hour, reflecting the value we place on our employees' contributions to delivering exceptional care and support services.
