Job Summary

The successful candidate will be responsible for delivering high-quality mental health services to patients within our trust. The focus will be on providing assessment, risk planning, and treatment plans to individuals with mental health difficulties and their carers.

Responsibilities

  • Deliver high-quality mental health services, including triage, assessment, and recovery-focused planning, referral, and signposting treatment to patients within the general hospital.
  • Patient and family/carer assessments, care planning, treatment, and reviews
  • Collaboration with multi-disciplinary teams to ensure consistent, well-coordinated care
  • Communication with primary care teams regarding client care
  • Facilitating fair access to social care services and promoting social inclusion regardless of age, ethnicity, gender, sexual orientation, or disability
  • Undertaking risk assessments and determining the appropriate course of action, taking into consideration various options
  • Providing and receiving information, some of which may be contentious, sensitive, and complex, to and from individuals and their families/carers, actively seeking support as needed
  • Developing personalized care packages based on bio-psycho-social assessments and working formulations

Requirements

  • An up-to-date professional mental health qualification (e.g., RMN, OT, or Social Worker) registered with no restrictions
  • Training to level 2 in Safeguarding children and young people: roles and competencies for Healthcare staff
  • Post-registration training in relevant fields and evidence of relevant continuing professional development

You will work in a supportive environment with a multidisciplinary team, where you will have opportunities to develop your skills and contribute to the delivery of high-quality services.

Salary

The salary for this role is approximately £25,549 per annum, based on the National Health Service (NHS) pay scales for a Band 5 clinical position.
