Adult Health Support Worker

The ideal candidate will be responsible for delivering high-quality patient care while ensuring the smooth operation of our care team. Key duties include:
• Providing one-to-one support to patients, tailoring care to their unique needs
• Collaborating with multidisciplinary teams to develop effective care plans
• Managing care assistant teams to maintain efficient workflows
Qualifications and Skills
To excel in this role, you will need:
• A Level 2 Diploma in Health and Social Care or equivalent qualification
• Strong understanding of dementia care principles and best practices
• Excellent communication and interpersonal skills to engage with patients and staff
• Ability to work independently and as part of a team, prioritizing tasks as needed
