The successful candidate will work with patients in their own homes or in bedded facilities in the community, supporting registered occupational therapists and physiotherapists in delivering ongoing intervention for rehabilitation.
The role involves assessing and supporting individuals to facilitate safety and maximise independence in their daily lives. Interventions may include a range of assessments and rehabilitation activities, assistive equipment, advice, and information sharing. Excellent communication skills are required to ensure a collaborative approach to patient care from the multidisciplinary team involved.
Main Duties of the JobThe focus of the role is to assess and support individuals to facilitate safety and maximise independence in their daily lives. Interventions may include a range of assessments and rehabilitation activities, assistive equipment, advice, and information sharing.
The post holder will be required to hold a Foundation Degree in Professional Practice or an equivalent Level 5 qualification in a relevant subject area. The purpose of the post is to provide a high standard of therapy service working under the supervision of registered Occupational Therapists and Physiotherapists.

Person SpecificationEducation and Qualifications
- Current full UK clean driving license.
- Foundation degree in Health and Social Care or equivalent with therapy experience for B4 or NVQ III Diagnostic and Therapeutic Support or experience and willingness to work to foundation degree for B3.
- Level 2 literacy and numeracy (GCSE Grade C or above in Maths and English).
Desirable Criteria
- IT related skills or training in Direct Care.
- Previous experience in specialist area of post.
- Other training related to post or clinical area such as Child Protection, Orthotics, sports and leisure.
- Previous experience of multidisciplinary and/or multi-agency working.
Occupational Experience
- At least 2 years working as a OT or physiotherapy assistant or at least 5 years' relevant work experience.
Management
- Able to manage own caseload.
