Job Overview

We are seeking an experienced Therapy Assistant to join our Community Stroke and Neuro team at Diana, Princess of Wales Hospital in Grimsby. This role involves working with patients in their own homes or in bedded facilities in the community for early support. You will support registered occupational therapists and physiotherapists in carrying out ongoing intervention for rehabilitation, and will carry your own caseload of cases following appropriate triage.

Interventions can include assessments and rehabilitation activities, assistive equipment, advice, and information sharing. Excellent communication skills are required to ensure a collaborative approach to patient care from the multidisciplinary team involved.

The post holder will be required to hold a Foundation Degree in Professional Practice or an equivalent Level 5 qualification in a relevant subject area.

Main Duties of the Job
