Job Title: Wellbeing and Transitions Outreach Worker

As a Wellbeing and Transitions Outreach Worker, you will be part of our Community Health and Prevention team, working to support young people's Emotional Wellbeing and mental health across various strands.

This is an exciting opportunity to join a well-established and respected team of Wellbeing and Transition workers, delivering Emotional and issue-based Wellbeing support to children and young people on a 1:1 and groupwork basis in Schools, on an outreach basis across Leeds and Wakefield.

We are looking for people passionate about supporting young people's emotional wellbeing who have experience in delivering health and wellbeing support to young people. You will be required to work restoratively to support a caseload of vulnerable young people and work closely with Schools, parents and/or other agencies and sometimes commissioners directly.

Key responsibilities include:

  • Delivering wellbeing support in Primary and Secondary Schools – 1:1 support and/or Group Work Programmes
  • Transitions – support/intervention for young people experiencing complex mental health (including self-harm and suicide ideation) who may or may not have had intervention from CAMHS
