About the Role

We are seeking a highly skilled and compassionate professional to join our team as a Memory Service Specialist. As a key member of our multidisciplinary team, you will work closely with older adults, their families, and carers to provide personalized care and support.

Key Responsibilities

  • Assess and plan care for older adults with memory difficulties, working collaboratively with colleagues to develop and implement effective treatment plans.
  • Provide cognitive testing, assessment, and diagnosis in the community or hub base, using evidence-based tools and techniques.
  • Deliver group and one-to-one education interventions to support service users and their families, promoting independence and well-being.
  • Maintain and promote a personalized approach in all aspects of service delivery, ensuring that each service user receives tailored care and support.
  • Work collaboratively with colleagues to coordinate the daily running of the Case Management Team, ensuring effective communication and seamless care delivery.
