Job Summary

We are seeking a highly skilled and compassionate individual to join our team as a Memory Service Assessor. As a key member of our multidisciplinary team, you will work closely with older adults who are experiencing memory difficulties, providing personalized care and support to ensure the best possible outcomes.

Main Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans to address the unique needs of each service user.
  • Provide cognitive testing and assessment in the community or hub base, using evidence-based tools and techniques.
  • Deliver group and one-to-one education interventions to promote healthy aging and dementia awareness.
  • Maintain and promote a personalized approach in all aspects of service delivery, ensuring that service users and their significant others are at the center of care.

Person Specification
  • Registered Nurse Mental Health or Occupational Therapist, with a minimum of 2 years post-grad experience.
  • Working knowledge and understanding of older adults with organic mental health conditions.
  • Ability to demonstrate sound knowledge and application of up to date evidence-based clinical practice in dementia care.
