B6 Assessor

We are seeking a highly skilled and dedicated B6 Assessor to join our Memory Service team at Sheffield Health and Social Care NHS Foundation Trust.

• Assess, plan, implement, and evaluate treatment and collaborative care planning for service users with memory difficulties.
• Provide cognitive testing and assessment in the community or hub base, working closely with service users and their supporters.
• Deliver group and one-to-one education interventions to support service users and their families.
• Maintain and promote a personalized approach in all aspects of service delivery, ensuring that service users and their significant others are at the centre of care.

• Registered Nurse Mental Health or Occupational Therapist with 2 years post-graduate experience.
• Working knowledge and understanding of older adults with organic mental health conditions.
• Ability to demonstrate sound knowledge and application of up to date evidence-based clinical practice in dementia care.
• Leadership and interpersonal qualities, with effective time management skills.
• Use of a car for work-related purposes.
