B6 Assessor

To provide cognitive testing, assessment, diagnosis and review in the community/hub Coordinate and actively participate in the daily running of the Case Management Team in the absence of the substantive post holder Work collaboratively with peers to co-ordinate the duties and responsibilities of other nursing and support staff within the service on a day to day basis Actively contribute to the development and delivery of therapeutic/treatment plans and psychosocial/educational interventions Ensure, through supervision, that each allocated service user has a comprehensive, clear and up to date collaborative care plan with specified objectives designed to meet the individuals identified care needs, ensuring effective clear communications and service user/significant others involvement in the formulation, while respecting the wishes and autonomy of the individual Participate and input into the multi-disciplinary team meetings [MDT] providing follow up as necessary/indicated post MDT Identify and make appropriate referrals to other agencies/services to support service users needs Provide support and guidance to junior staff members Develop and maintain effective working relationships with Primary Care colleagues and 3rdparty sector Participate with colleagues in the promotion of an active learning environment in order to maintain and develop high standards of professional and clinical practice. Promoting opportunities for self, nursing colleagues and support staff to access available education/training.
