Community Physiotherapist

We are seeking a skilled and experienced Physiotherapist to join our team as a Locum Community Physiotherapist in an Adults Community setting.
Key Responsibilities:- Assess and manage patients in an Intermediate Care setting, providing community physiotherapy and rehabilitation services.
- Work collaboratively with interdisciplinary teams, demonstrating awareness of holistic approaches to assessment and care.
- Develop and implement individualized treatment plans, promoting patient independence and well-being.
- Conduct home visits and provide physiotherapy services in community settings, as required.
- HCPC registration and a valid driving license.
- Experience in community physiotherapy and rehabilitation, with a strong understanding of Intermediate Care principles.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
