About the Role

We are seeking a highly skilled and motivated registered nurse to join our expanding Frailty team at Calderdale and Huddersfield NHS Foundation Trust. As a specialist nurse in Acute Frailty, you will play a vital role in providing high-quality care to our frail patients.

Key Responsibilities
  • Act as a specialist clinician for the Acute Frailty Service, working autonomously within Trust policies and professional practice.
  • Manage your own specialist caseload, demonstrating excellent skills in effective prioritisation and maintaining accurate and comprehensive records.
  • Provide and develop the core components of the specialist Frailty nurse role, including clinical practice, management, and education.
  • Support the Team Leader in the day-to-day running of the Acute Frailty service and contribute to the wider multidisciplinary team.
  • Participate in the assessment, planning, implementation, and evaluation of individualised programmes of care for frail older adults.

Requirements
  • First-level registration with the NMC.
  • Post-basic qualification in a relevant specialty.
  • Demonstrable experience as a Band 5 nurse in an elderly nursing environment.
  • Experience working in an Acute environment and palliative care.
