Lymphoedema Specialist Practitioner

We are seeking a skilled and compassionate professional to join our team as a Lymphoedema Keyworker at St Margaret's Somerset Hospice. As a key member of our multidisciplinary team, you will provide expert care and support to patients with lymphoedema, working closely with our Lymphoedema Specialist to ensure seamless service delivery.
Main Responsibilities- Provide comprehensive assessments and treatment plans for patients with primary and secondary lymphoedema, including those with life-limiting conditions.
- Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare professionals to optimize patient outcomes.
- Deliver high-quality care and education to patients, families, and carers, promoting self-management and empowerment.
- Contribute to the development and implementation of lymphoedema services, ensuring best practice and evidence-based care.
- Collaborate with the multidisciplinary team to ensure seamless service delivery and patient-centered care.
