Job Overview

We are seeking a highly skilled and compassionate Dementia Care Specialist to join our team at Derbyshire Community Health Services NHSFT. This is an exciting opportunity for someone who is passionate about providing exceptional care to individuals with dementia.

About the Role

In this role, you will be responsible for delivering high-quality therapeutic occupational and physiotherapy activities to patients with dementia and associated problems related to the aging process. You will work under the guidance of registered staff to assess the social, psychological needs of patients by undertaking designated assessments in activities of daily living, cognition, and mood.

This is a challenging yet rewarding role that requires empathy, understanding, and excellent communication skills. As a Dementia Care Specialist, you will play a vital part in promoting patient well-being following a 'Person-Centred Approach' and supporting the therapy and nursing teams in implementing high standards of patient care.

Main Responsibilities

Your key responsibilities will include:
  • Delivering personal care duties for patients consistent with their agreed plan of care while maintaining/promoting independence, privacy, and dignity.
  • Undertaking programmes of care to maximise/maintain patients' independence.
  • Working as a member of a multi-disciplinary team on behalf of patients, assisting with coordinated care on patients' admission, care period, and planned discharge.
  • Planning and organising your routine day-to-day tasks and activities.
  • Participating/initiating/preparing social and therapeutic interventions with patients to achieve a planned programme of care and a stimulating environment.
  • Recording clinical observations or test results personally undertaken and collecting samples for investigative purposes.
  • Receiving and giving reports to the therapy/nursing team regarding patient progress, condition, and treatment.
  • Working in an environment with distressing/emotional circumstances.


Requirements

To be successful in this role, you will need:
  • Able to demonstrate knowledge of how to meet the needs of older adults with mental health problems.
  • Demonstrate knowledge and understanding of the importance of participation of activities on health and well-being.
  • Experience in caring for older adults with dementia.


Salary and Benefits

The salary for this role is £24,907 - £30,615 per annum, depending on experience, plus benefits including a contributory pension scheme, annual leave entitlement, and opportunities for professional development.
