Job Title: Community Medicine Specialist

We are seeking a highly skilled and experienced Community Medicine Specialist to join our dynamic team at the Transformation Unit.

About the Role

The successful candidate will be responsible for providing high-quality medical care to older adults in our community hospitals and residential intermediate care facilities. This will involve working closely with our multidisciplinary team to deliver comprehensive geriatric assessment and rehabilitation services.

Main Responsibilities
  • Providing acute medical care to older adults in our community hospitals and residential intermediate care facilities
  • Delivering comprehensive geriatric assessment and rehabilitation services
  • Working closely with our multidisciplinary team to ensure seamless care coordination
  • Developing and implementing individualized care plans to meet the unique needs of each patient
Requirements
  • Eligibility to register with the GMC
  • MRCP or equivalent qualification
  • Completed foundation and IMT training
  • Wide range of experience in acute and OPM
  • Experience of orthogeriatric medicine
