Care Coordinator

The ideal candidate will have experience working with individuals with learning disabilities and autism, and will be able to provide support and care that promotes independence, dignity, and well-being. As a Support Worker, you will work as part of a close-knit team to deliver person-centred care that meets the individual needs of each resident.
You will be responsible for providing assistance with daily living tasks, such as personal care, meal preparation, and medication management. You will also work closely with other healthcare professionals to develop and implement care plans that promote the health, safety, and well-being of each resident.
