We are seeking a Care Coordinator to join our team at Target Point New Srl in Sheffield. The successful candidate will be responsible for providing intensive housing management support to vulnerable individuals, promoting client independence and delivering measurable outcomes.

Job Summary

The role of the Care Coordinator involves engaging with referral agencies to generate referrals, undertaking assessments for potential residents, and completing documentation to support tenancies. Additionally, the coordinator will design and deliver effective support plans, engage with tenants, and develop professional relationships with external agencies.

This is an exciting opportunity to work with a well-established and expanding supported accommodation project. As a Care Coordinator, you will have the chance to make a real difference in the lives of vulnerable individuals and contribute to creating a supportive community.

We offer a competitive salary of £25,115 per annum, rising to £26,425 after 12 months and £27,355 after 24 months (pro rata). Our benefits package includes mileage compensation, membership to Westfield Health, access to Lifetime Financial, and 28 days annual leave (plus statutory bank holidays).

Key Responsibilities:

  • Engage with referral agencies to generate referrals
  • Undertake assessments for potential residents and review as required
  • Complete documentation, including tenure agreements and property inventories
  • Design and deliver effective support plans for tenants
  • Maintain contact with tenants and develop professional relationships with external agencies

Requirements:

  • Experience of working with vulnerable individuals in a housing environment
  • Ability to develop constructive relationships with offenders
  • Knowledge of the Welfare Benefits System
  • Basic IT skills suitable for an office environment
