Exciting Opportunity in Community Nursing

We are seeking a skilled and dedicated Assistant Practitioner to join our Community Nursing Team in the Pembroke District. As an Assistant Practitioner, you will play a vital role in supporting the management of a delegated caseload, providing high-quality care to patients in their own homes.

The ideal candidate will have significant experience in providing relevant clinical care, including wound care, administration of medication, and venepuncture. They will also possess excellent interpersonal and communication skills, with the ability to manage and diffuse challenging situations.

Key Responsibilities:

  • Deliver high standards of care to patients and their families/carers within the home or clinic environment
  • Liaise with staff, patient/carers, and other agencies/organisations to ensure appropriate delegation of duties and risk management
  • Manage daily caseloads effectively, recognising unwell patients and escalating care accordingly

Requirements:

  • Level 4 vocational qualification in a relevant health care associated subject or Certificate in Health Care Support Worker Education
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
  • Evidence of continuing personal development relevant to the role in health care
  • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the all Wales Band 4 Assistant Practitioner Governance Framework
  • Clinical knowledge relevant to the role
