We are seeking a dedicated and compassionate Care Coordinator to join our team at Sonderwell Staffing Solutions. As a Care Coordinator, you will be responsible for providing support and care to adults with various learning disabilities, promoting their independence and overall well-being.

About the Role

This is a challenging yet rewarding role that requires a strong understanding of the needs and requirements of individuals with learning disabilities. You will work closely with our clients to develop personalized support plans, ensuring their physical, emotional, and social needs are met.

As a Care Coordinator, your day may include tasks such as:

  • Assessing clients' needs and developing tailored support plans
  • Coordinating with healthcare professionals and other service providers to ensure comprehensive care
  • Providing guidance and support to clients in areas such as personal care, medication management, and daily living skills
  • Maintaining accurate records and reporting on client progress

To be successful in this role, you will require excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. A strong understanding of the principles of person-centered care and a commitment to supporting individuals with learning disabilities are essential.
