Job Summary

We are seeking a highly skilled and experienced Occupational Therapist to join our team at Central London Community Health Trust. As a Community Occupational Therapist, you will play a vital role in providing comprehensive, holistic assessments and treatments to patients in their own homes and community settings.

Key Responsibilities
  • Conduct thorough assessments and develop individualized treatment plans to maximize patient independence and well-being.
  • Work collaboratively with multidisciplinary teams, including physiotherapists, nurses, and social workers, to ensure seamless care coordination.
  • Prescribe and provide equipment and adaptations to patients to enhance their quality of life.
  • Travel to patients' homes and community settings to deliver assessments and treatments.
  • Supervise and mentor junior staff and students to ensure high-quality care and professional development.
  • Participate in continuing professional development and contribute to the development of the service through research and audit.
Requirements
  • Degree or Diploma in Occupational Therapy.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Minimum 2 years post-graduate experience in community occupational therapy.
  • Experience working as an autonomous practitioner, assessing and treating patients with complex medical conditions.
  • Ability to apply core occupational therapy skills across a range of clinical conditions with a client-centered goal setting approach.
  • Knowledge and understanding of complex medical conditions, including multi-pathologies and falls.
Desirable Qualifications and Experience
  • Postgraduate qualification in occupational therapy or a related field.
  • Experience of community working and working with adult and/or elderly client groups.
  • Experience in supervising staff and students.
  • Knowledge and experience in research and audit.
Personal and Physical Attributes
  • Confident to work as an autonomous practitioner, alone in community settings.
  • Flexible, adaptable, and reliable.
  • Physically fit to perform the job description requirements with or without adaptations.
  • Able to drive and have a car to use for work, with a clean driving license.
