We are seeking a highly skilled and dedicated Acute Care Qualified Practitioner to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our Home Treatment Team, you will play a vital role in providing intensive and enhanced support to individuals with Autism and/or Learning Disabilities.
Working closely with our Crisis Resolution and Home Treatment Service, you will be responsible for coordinating care needs, assessing, planning, and implementing evidence-based care to service users diagnosed with a Learning Disability and/or Autism. Your expertise will be invaluable in facilitating admission avoidance and ensuring the prevention of unnecessary hospital admission.
Key Responsibilities- Provide high-quality clinical care, ensuring safe and effective assessment, treatment, and comprehensive discharge planning.
- Assess, treat, and support individuals who display challenging/distressed behaviors, utilizing a bio-psychosocial approach and collaboration with other professionals.
- Deliver urgent support in response to crises, aiming to prevent family/service breakdown, inpatient admission, or out-of-area placements.
- Rapidly assess and liaise with care commissioners and providers to modify care and support, reducing the risk of harm/placement breakdown.
- Support individuals across various community settings, including their own homes, family homes, short break services, and residential placements.
- Offer time-limited direct and intensive support, including hands-on support for paid carers/family carers.
- Operate flexibly across a 7-day week, delivering care jointly with other Crisis Resolution & Home Treatment staff.
We are looking for a candidate with:
- A qualified Registered Mental Health/Learning Disability Nurse or Social Work qualification.
- Active registration on the appropriate professional register (NMC, etc.).
- Evidence of specialized continued professional training (degree level) in clinical practice.
- Relevant experience of working with adults and/or children/young people with a Learning Disability and/or Autism.
- Relevant experience of working with adults with mental health needs or clear evidence of substantial experience working with individuals in crisis and transferable skills.
- Good understanding of community resources.
