About the Role

We are seeking a highly skilled and compassionate Acute Care Practitioner to join our Crisis Resolution and Home Treatment Team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to individuals experiencing acute mental health crises.

Key Responsibilities
  • Conduct comprehensive clinical assessments to identify the care needs of service users presenting with acute mental health crises or symptoms of mental distress.
  • Develop and implement evidence-based care plans to ensure identified care needs are met, enabling service users to transition from crisis point to discharge.
  • Provide professional advice and support to other agencies and members of the multi-disciplinary team.
  • Work collaboratively with colleagues to promote effective working relationships and ensure effective clinical decision-making.
  • Maintain accurate and timely clinical records and coordinate those of the team.
Person Specification

We are looking for a candidate with:

  • Registration as a Registered Nurse, Mental Health Nurse, Social Worker, or Occupational Therapist.
  • Experience of working with adults with mental health needs, particularly those with severe and enduring mental illnesses.
  • Ability to travel independently throughout the county.
  • Strong communication and interpersonal skills.
