In this role, you will be responsible for delivering personalized care to patients in their homes or residential settings, while also working in clinics when necessary. Your key duties will involve:

  • Identifying and addressing the unique needs of each patient, prioritizing their care accordingly
  • Managing symptoms related to palliative and end-of-life care
  • Coordinating catheter care and other medical interventions
  • Providing expert wound care and management
  • Developing and implementing effective care plans to manage chronic conditions

For this position, you will require:

  • A current NMC registration as a general nurse (holding a degree in nursing)
  • A valid UK driver's license and reliable access to a vehicle
  • Relevant experience within the healthcare sector, preferably in community-based settings
  • Demonstrated expertise in care planning, information governance, and other relevant areas
