Job Description

We are seeking a highly skilled Clinical Diabetes Care Coordinator to join our team. The successful candidate will be responsible for working with primary care teams to deliver high-quality care to patients with diabetes. This will involve:

  • Collaborating with healthcare professionals to develop and implement care plans
  • Providing education and support to patients and their families
  • Maintaining accurate records and reports
