Job Summary

We are seeking a highly motivated and forward-thinking Clinical Pharmacist to join our team at Swaffham & Downham Primary Care Network. As a Clinical Pharmacist, you will play a key role in developing and implementing medication optimisation strategies to improve patient outcomes.

Main Responsibilities

Key responsibilities will include:

  • Performing clinical medication and long-term condition reviews to address medication-related issues and optimise medication.
  • Providing specialist knowledge on public health programmes and supporting public health campaigns.
  • Supporting general practice teams to ensure compliance with CQC standards and implementing changes to medicines that result from MHRA alerts and local and national guidance.
  • Undertaking audits of prescribing in areas directed by GPs and making recommendations for improvements.
  • Providing educational and training to practices on therapeutics and medicines optimisation.
  • Identifying national and local policy and guidance that affects patient safety through the use of medicines and implementing changes accordingly.
  • Supporting the PCN with the delivery of QOF, IIF and other targets as driven by NHS England and Norfolk & Waveney ICB.

Person Specification

We are looking for a Clinical Pharmacist with:

  • A Degree in Pharmacy and GPHC registration.
  • Independent prescriber qualification.
  • Minimum of two years post-graduate experience in secondary, primary or community pharmacy.
