Expert Diabetes Care Provider

NSHI Ltd is seeking a highly skilled Diabetes Specialist Nurse to join our nationwide team of full-time Diabetes Specialist Nurse Advisors working in primary care.
Key responsibilities include delivering high-quality care to improve patient outcomes, in line with local and national Diabetes guidelines (NG28). This involves working proactively to identify sub-optimally controlled patients with Type 2 diabetes, optimising their oral therapy, conducting point of care testing, and referring for specialist insulin treatment when necessary.
Essential requirements include a Diabetes diploma or equivalent, a minimum of 3 years' experience managing diabetes within primary care, and knowledge and confidence in providing education, support, and advice to people with Type 2 Diabetes.
