Job Summary

A rare opportunity has arisen within the Adult Community Learning Disability team for a Band 6 Community Practitioner to join our team working within our Specialist Behaviour pathway.

Our team is made up of a wide range of multidisciplinary professionals with a friendly specialist approach to supporting individuals with a learning disability maintain independence within the community. We aim to overcome health inequalities and promote wellbeing, enhancing the lives of our patients.

Main Duties of the Job

To manage case work under the direction of a care coordinator/pathway clinical lead and provide ongoing assessment and recovery-focused care planning for service users who do not have enhanced needs and where risks are assessed as low. Under the supervision of the pathway clinical lead, provide effective treatment and intervention to service users with complex and substantial needs requiring support from our Specialist Behaviour team.

Deliver evidenced-based clinical and psychologically-based interventions, with appropriate training and supervision, as directed by the pathway clinical lead. Work as a member of the multidisciplinary team to ensure high-quality, individualized care is delivered, promoting recovery and wellbeing, and maximizing independence.

Provide compassionate care that is based on empathy, kindness, respect, and dignity.

Person Specification

Education and Qualification

  • Professional qualification and current professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.

Desirable

  • Experience of working within a community team.
  • Non-Medical Prescribing.
  • Additional training/qualifications in positive behaviour support.

Knowledge and Experience

  • 12 to 24 months of band 5 qualified nurse experience, dependent upon service need.
  • Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.

Skills & Competencies

  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management/Safety Planning skills.
  • Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches as identified in guidance.
  • Demonstrate effective written and verbal communication skills.
  • Evidence of a high level of professional, organisational, and leadership skills (self and others).

Desirable

  • Clinical leadership/delegation skills.
  • Ability to use electronic recording and relevant IT equipment.
  • Proven experience of acting as an effective advocate for service users and their carers.
