About You

  • A degree or equivalent in social work.
  • Registration with Social Work England.
  • Proven experience in assessing needs, developing care plans, and providing person-centred support.

Key Responsibilities

  • To work collaboratively with service users, carers, and other professionals to deliver holistic care and support.
  • To conduct thorough assessments of service user needs and develop tailored care plans.
  • To provide ongoing support and review care plans to ensure effective delivery.

Salary and Terms

  • An attractive hourly rate of £35 per hour (assignment rate), plus a range of benefits.
