Job Overview

We are seeking a highly motivated and experienced Mental Health Practitioner to join our dynamic Community Mental Health Team. As a key member of the team, you will provide high-quality, compassionate care to older adults with dementia and functional mental health conditions.

Key responsibilities include conducting comprehensive assessments, developing personalized treatment plans, and providing support to patients and carers. Main Duties of the Job

Key duties include:

  • Undertaking assessments and coordinating care for service users
  • Working closely with family members to maintain high standards of collaborative care
  • Contributing to the day-to-day operational functions of the team, including completing new assessments and covering office duties
  • Developing and maintaining positive working relationships with service users, carers, GPs, and other professionals
  • Ensuring clear channels of communication are established and maintained with clients, carers, GPs, and other partners

Person Specification

Key qualifications and experience include:

  • Registered with the appropriate professional body
  • Professional knowledge acquired through a relevant degree plus relevant experience for working with complex cases
  • Minimum of 2 years post-registration experience working with adults with mental health problems
  • Experience of planning and organizing complex activities or programmes requiring formulation and adjustment
