Key Responsibilities:

  • Undertake comprehensive assessments and develop personalized treatment plans for service users.
  • Work collaboratively with the multidisciplinary team to deliver high-quality care and support to service users and their carers.
  • Develop and maintain positive working relationships with service users, carers, General Practitioners, and other healthcare professionals.
  • Contribute to the development of services in the Cheshire East area and take a lead role in the development of services in conjunction with the Team Operations Manager and other professionals.
