We are seeking a highly motivated and experienced Mental Health Practitioner to join our dynamic Community Mental Health Team. As a key member of the team, you will provide high-quality, compassionate care to older adults with dementia and functional mental health conditions.
Key Responsibilities:
- Undertake comprehensive assessments and develop personalized treatment plans for service users.
- Work collaboratively with the multidisciplinary team to deliver high-quality care and support to service users and their carers.
- Develop and maintain positive working relationships with service users, carers, General Practitioners, and other healthcare professionals.
- Contribute to the development of services in the Cheshire East area and take a lead role in the development of services in conjunction with the Team Operations Manager and other professionals.
